About Our Growing Company

Coordinated Care Services (CCS) aims to provide consistent, quality, in-home and community support services with people who are ageing or living with a disability, allowing them to maintain independence, individuality, dignity and quality of life. Our friendly, experienced and professional staff will help achieve this in a safe and supportive environment, with the person and their family at the centre of everything we do.

CCS envisions a society where all people are treated with equality, dignity and respect for their expressed choices and who have equal opportunities to participate in a society where abilities, rather than disabilities, are recognised.

CCS offers a range of services, including assistance with daily living, nursing care, social support, transport assistance, appointment support, grocery maintenance, gardening, dementia care, including implementation of specialist care plans developed by clinical professionals, brain gym and brain training, as well as positive behaviour support planning.

CCS are committed to working with the person and their families to design a service, including a staffing model, with speciality training, tailored to individual needs and requests. CCS is also committed to a collaborative approach and works together with many different agencies across the ACT and NSW to ensure a holistic, safe and person centred service is applied.

This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and achieve your goals
Support Coordination
We work with you to strengthen your ability to coordinate supports and participate in the community.
Assisting with-Life Stages / Transition
We offer short-term or ongoing support that strengthens your ability to live an independent life and participate actively in your community. This help can be available by connecting with providers and community groups.
Community Participation
This is an individualised service (for both adults and children) to assist you to complete daily activities that are relevant to your individual needs and support requirements. Supports we can provide include cooking, shopping, attending appointments with you or in home support such as lawns and garden maintenance
Assisting with Personal Activities
We can assist you to with skills including knowing how to be part of a team, manage your money, manage your time, live as part of a family, building relationships and effective communication.
Development / Life Skills 
Daily tasks are those that fall under the same category as household tasks and personal care activities. Our team is as useful with daily task management within many living environments included shared living.

NDIS innovative community participation programs are mainly designed to provide NDIS participants like you, a chance to participate in social activities or welfare scheme. Such programs are targeted towards improving your well-being, in your personal life and careers (employment, health, education, income, and life satisfaction outcomes). Innovative community participation can lead to improved well-being outcomes
