About Us

Coordinated Care Services (CCS) aims to provide consistent, quality, in-home and community support services with people who are ageing or living with a disability, allowing them to maintain independence, individuality, dignity and quality of life. Our friendly, experienced and professional staff will help achieve this in a safe and supportive environment, with the person and their family at the centre of everything we do.

CCS envisions a society where all people are treated with equality, dignity and respect for their expressed choices and who have equal opportunities to participate in a society where abilities, rather than disabilities, are recognised.

CCS offers a range of services, including assistance with daily living, nursing care, social support, transport assistance, appointment support, grocery maintenance, gardening, dementia care, including implementation of specialist care plans developed by clinical professionals, brain gym and brain training, as well as positive behaviour support planning.

CCS are committed to working with the person and their families to design a service, including a staffing model, with speciality training, tailored to individual needs and requests. CCS is also committed to a collaborative approach and works together with many different agencies across the ACT and NSW to ensure a holistic, safe and person centred service is applied.

Martin Director (DIR)
Martin has variously completed a BA in History and Government from the University of Adelaide, and a Masters Degree from the University of Sydney in Health Sciences (Developmental Disability). Martin is also a certified Bookkeeper which helps navigate funding models.
Bron General Manager (GM)
As a former support worker and scheduling officer. Bron brings her years of experience to Coordinated Care Services (CCS).
Tenille Scheduling Coordinator (SC)
Tenille started as a Social Worker in Goulburn and worked her way up to the position of Scheduling coordinator, and works Mon, Wed, and Fri in the office. Tues and Thurs she covers social work in Goulburn and surrounding areas.

Courtney is a Social WorkerW from Goulburn. She enjoys spending time with her children, going to the Gym, hanging out with friends etc. she is studying cert IV Mental Health as well as this is her interest.

Sujan is a social worker from Queanbeyan. He enjoys spending time with his family and friends and also runs a private driving school in his free time during the week. Sujan specialises in Down Syndrome care for NDIS participants

Annalea is a support worker based in Queanbeyan who works with a variety of participants. Annalea is caring and empathetic and is willing to give anything a go. Annalea works with people of all ages and diversity.

Hikmat Support Worker (SW)
Hikmat works exclusively iin the Canberra region. Hikmat has a wealth of experience in the disability sector and is a Down to earth person. He is available from 5pm Monday to Friday, and occasional weekends.

Paribesh Accounts
Paribesh is responsible for managing and the reporting of financial information of our organisation. He has completed Masters in Professional Accounting(MPA) and Masters in Business Administration(MBA) from Holmes Institute- Sydney Campus. Paribesh has a Diverse Experience as a Accounting, Banker, Customer service, Security officer and Chef. He loves creating, scaling and optimizing business portfolios through focusing on delivering key outcomes and adapting new learning.
